Our support ranges from simple, practical assistance with everyday tasks like cooking and cleaning, to personal care, such as helping clients bathe, dress, and take their medication. Our care professionals help administer medication, provide nursing support, or help facilitate meet-ups with friends and family. They accompany clients on day trips and help them maintain social lives as well. We also have specially-trained staff to assist with age-related conditions like dementia and Parkinson’s disease.
Our services are bespoke to each client, pairing a care professional or support worker according to their skill-sets and a client’s particular needs. Each care plan is designed around a specific client’s requirements. We set out key objectives at the start of a care programme to measure successes against at a later stage. At the end of the day, we’re also supplying a companion to our clients, and we strive to insure a perfect match.
Physical & learning disabilities
Facing disability doesn’t mean losing out on all life has to offer. The care professionals at Help at Home provide support and care that focuses on independence, promotes activity, and encourages a social life wherever possible. Clients are then able to live a happier life within the comfort of their own homes.
We assist clients with the full range of daily living tasks while they’re in our care. Whether personal care, meal preparation, laundry, shopping, housework, or managing finances, we help our disabled clients lead active lives in and out of the home.
Care plans are designed around our clients’ preferences and aspirations. This person-centred, outcomes-focused approach gives our clients more choice and control over when and how they receive the support they need.
It’s not only adults that we help. We also provide support to many children and young people, helping them have the best start in life they can. We do what we can to offer them as fun-filled and active a childhood as possible.
We utilise person-centred approaches to supporting people with physical and learning disabilities so they can make as many decisions for themselves as possible. If our staff are required to make choices on a client’s behalf, we always look to do so in their best interests. It’s vital, too, that we match the right care professional with the right client based on respective skill-set and care needs.
Live-in care
For many, maintaining independence at home becomes too challenging when struggling with a disability or illness. Live-in care offers a level of support that allows an individual to continue enjoying life in the place they’re the most comfortable.
By opting for live-in care, a person still has control over their day-to-day routine, but with the added assistance of a care professional to help with general daily chores. Help at Home’s team are trained to aid in a wide range of household activities, from personal care to meal preparation. We also provide support with specific health-related requirements.
We’re well aware that live-in care can be a big adjustment. It’s our goal to make the process as easy and comfortable as we can for you or a loved one. This begins with finding a care professional who has the right experience and training. We also consider a care professional’s personality, which makes it easier for them to form a connection and a lasting relationship with the person in their care.
Reablement
Our trained reablement teams support people of all ages and needs to regain their skills and confidence at home, reducing or removing the need for ongoing care.
Getting back from a long hospital stay often means the loss of physical strength and confidence, a daunting prospect for anyone faced with that reality. Our expert reablement team works together with nurses, physiotherapists, and occupational therapists to assist in a client’s rehabilitation to partial or full recovery.
We’ll always look to encourage our clients to help and support themselves as much as they can. We set short and long-term goals that allow clients to take each day step-by-step in regaining independence. Supported by our experienced and highly-trained care professionals, clients will stretch their limits in line with agreed goals. Progress is closely monitored and their reablement plan will be adjusted as necessary to respond to the their changing abilities.
